1. PERSONAL INFORMATION: if you are satisfied with our trip arrangement, please provide us with the following informatione:

• Full name as shown on the passport;
• Date of Birth (required by all major airlines at the time of booking);
• Passport number;
• Mailing Address (your mailing details will enable your travel agent to better prepare the invoice);
• City of departure (this will let us find the best routes of travel for your international flight);
• Any special requests (i.e., diet, medical assistance and seating preferences, et cetera).

2. INSURANCE: Determine whether you need the travel insurance before paying the deposit. If you need China Travel Service (Canada)  Inc. to arrange the travel insurance with insurance company, you must provide us with the following information:

• Your full name as shown on your passport;
• Your date of birth;
• Your credit card information, to enable insurance company to issue the policy by charging the premium;
• Your selection of the type of insurance policy.

3. INTERNATIONAL AIR TICKET BOOKING: Once you have decided to join the tour, it is advised that you book the international air ticket ASAP. Doing so will enable your travel agent to find the lowest airfare for you. You will only need to provide your travel agent with your full name as shown on your passport and to issue the ticket within the certain period of time in accordance to the fare contract after booking; otherwise the booking will be cancelled by the airlines.

4. CREDIT CARD PAYMENT AUTHORIZATION FORM: You must complete your Credit Card Payment Authorization Form & provide your signature with it so that we may collect the DEPOSIT, enable the insurance company to prepare your travel INSURANCE policy, issue your international flight tickets and settle your full payment.

5. DEPOSIT: Without providing your travel agent with a deposit payment, he/she will not be able to dprocess your booking. In order to enjoy the programs and activities listed in the itinerary, enable your travel agent to book the listed hotels and cruise ship, and have quality services for the tour, you are required to settle the deposit as a final confirmation of the booking at your earliest conveniences.

6. APPLY THE CHINA VISA: You are required to apply for your visa for the destination country at least 6 weeks before departure. You may simply send [with FedEx or other registered mail] your original passport with two 1.5 x 1.5 inch photos to your travel agent. They will then happily take care of this on your behalf.
